The new accident insurance is under Guardian and begins 9-1-2023.  Claims should be made with them. Go to the Gentry Financial page and use the claim forms under Guardian Accident Insurance.


If you need to file wellness for METLIFE Accident (This insurance ended as of 8-31-2023), there is still time.  Calling Wellness Claims at 1-866-626-3705.  Ask for the Wellness Claim Department and have the following information available: 1. Date of service performed 2. Type of service performed (annual physical, annual eye exam, or annual dental exam  If this was an Annual doctors visit with a full physical & blood work, you will receive 2 separate checks for $100.00 each (Hospital Indemnity & Accident Wellness). If you file an Annual doctors visit for Dental, Vision, or a Mammogram, you will need the same information to give to them as listed above for a wellness claim but you will only receive 1 check for $100.00.  If you have any problems with your claims, please call Dr. Shannon Orsborn, Director of Human Resources @ ext: 8146.
